The following rules apply to our program which are open to any patient who refers a friend or family member to our practice.

1)      All existing patients who refer a new patient for any cosmetic or surgical procedure as detailed above are eligible for continued rewards which can be used for any skin care product, cosmetic or surgical procedure. (procedures covered under patients medical insurance are excluded)

2)      Rewards will be credited to the referring patient through a gift card which can be used at any of our offices. Credits issued do not expire and may not be transferred to another patient.

3)      The new patient must present a referral card to our office at the time of the initial office visit or consultation listing the name of the referring patient in order for the credit to be issued.

4)      The new patient must schedule (with advanced deposit) or perform the procedure at the initial office visit in order for the credit to be issued to the referring patient. Consultations without procedures performed do not apply.

5)      Credit will be issued for initial procedures for referred patients only. Subsequent procedures performed will not be entitled to additional credit.
